Transition Planning: From High School to Independence
Transition planning helps students with disabilities prepare for life after high school. By law, transition planning must begin by age 16, but it's never too early to start thinking about your child's future.
What is Transition Planning?
Transition planning is a coordinated set of activities designed to help students move from school to post-school activities, including:
- Post-secondary education (college, vocational training)
- Employment (competitive, supported, sheltered)
- Independent living
- Community participation
